Many have thought that the Pip-Boy as seen in Fallout 1, 2 and tactics, was a handheld device, sorta like a cross between a game-boy and a 80's cell phone in portability.

However. This is not backed by lore. The Vault Dweller's Survival guide, intended to be an in-universe guide. Explictly references it as wrist worn.

The misconception came about due to an unsourced claim on the wikia, that the wrist-mounted device seen in the game was infact a separate edition unofficially refered to as 2000 plus. This claim was left unsourced, but is infact based on JE Sawyer's pen and paper rpg. which he was devising during the development of Van Buren. The RPG was never completed, and never canon. Thus there are no known Pip-Boy versions that are wrist-mounted.

Unfortunately there is no canon portrayal of what the full thing is supposed to look like, besides it's main face and it's screen. But we can probably assume something similar to the Pip-boy 3000. Or perhaps a more convenient version of the pip-boy 1.0, seen in Fallout 4's intro Which is very bulky, but seems to be designed to be reminiscent of the pip-boy 2000 menu.

As for why the Pip-boy has always been wrist-mounted. I would guess it has to do with how it tracks the health of the user.
